Inexperienced Workers and NPTC Provisional FMO Certificates
Two recent serious incidents have involved people working with NPTC provisional FMO certificates. The individuals had still not been assessed as competent, but were working without supervision.
Lack of experience and lack of supervision contributed to the accidents that followed.
Cablecrane Carriages, Chains and Communications
A recent incident resulted in serious injuries to a chokerman’s hand, while working with a skyline. A full investigation is still underway. However, the investigation has identified 3 areas of skyline work that can contribute to increased risk of injury and they are the subject of this alert.
Winches and Guarding
A recent legal judgement as a result of an accident, has clarified the requirement for guarding and safe working positions where there is a risk of being entangled in the moving parts of winching machinery.
An example is the Syncrofalke Skyline System similar to the one being used in the photograph opposite, manufactured in Austria, with a CE mark but without guarding for the main cable drum.
